Objective: To highlight the mother's perception of the signs of pain in a newborn with congenital heart disease in a neonatal intensive care unit using a scale of behavioral and physiological signs - the Neonatal Infant Pain Scale. Method: This is a descriptive study with a qualitative approach. The research was carried out from October to November 2018, with 8 mothers of newborns with heart disease in the neonatal ICU of a Public Hospital of Reference in Cardiology in Belém-Pará. The data collected was analyzed based on Bardin's content analysis. Results: Three thematic categories emerged from the analysis: The mother's understanding of the presence of pain in the cardiac neonate admitted to the NICU; The behavioral and physiological manifestations related to pain in a cardiac neonate perceived by the mother in the NICU; The painful procedures experienced by the neonate perceived by the mother. Conclusion: The contribution of this research is intended to favor the multidisciplinary team's compression, delving deeper into the dimensioning of pain in cardiac neonates in the perception of the mother in the NICU.
